Elders Support Worker – Permanent
Department: Health & Social

Standard Hours Bi-Weekly: 75 Full-time
Start date: Immediately
Salary: Level 7 Step 1(73,272.14) Annually

Reporting to the Director of Health and Social, the Elders Support Worker is responsible for the effective implementation, management and administration of the TH Elders Support Program. The range of services and supports include; home visits, assistance navigating programs and services, advocating for their needs, arranging travel, providing relevant supports for appointments, outings, meetings and events, addressing enquiries and concerns, providing explanations and resource information, liaising with family members, planning of social, cultural and recreation activities, logistical support to the Elders Council, funding proposals for special programs and events, assists with strategies to help manage chronic conditions and serves as a member of the Health & Social case management team to ensure that Elders receive the best possible program supports.

Essential Qualifications:

· Grade 12, and preferably with a post-secondary diploma in a social or health care discipline, and two years of experience in delivering programs and services to seniors / elders, and;
· Knowledge of current practices in health promotion, basic chronic disease prevention and management, and community health care support programs and services;
· Knowledge of Elders benefits programs including financial assistance supports;
· Or an equivalent combination of education, training, skills and experience.

Assets:

· Knowledge of Tr'ondëk Hwëch'in government structure, culture, traditions, history, demographics, family structures, socio-economic dynamics, challenges and aspirations, basic phrases of the Hän language, as well as the intergenerational impacts of residential schools on individuals, families and communities.

Conditions of Employment:

· Mandatory confidentiality is a condition of employment for all Tr'ondëk Hwëch'in personnel. Failure to meet this requirement could result in dismissal.(same for all positions)
· Criminal Record Check;
· Driver's license with clear abstract;
· First aid.
